2010 Scholarship Application

Instructions

In order to file an online application, you must complete the application in one sitting (about 20 minutes-if you have all of your information ready). You cannot quit part way through and resume at another time. If you think that you will need to stop before completing the form, you should print the form, fill it out offline, and submit the paper copy to the EHS guidance office. The 2010 deadline for applications is Friday, April 2nd. All materials, including application form, recommendations, appraisals, and any supplemental materials for specific scholarships must be turned in to the EHS Guidance Office by this date.

Please complete this application so we can determine your eligibility for receiving funds set aside to help students who plan to go on to postsecondary education, and who satisfy other criteria developed by EHS Dollars for Scholars and scholarship award providers. Filling out this application and submitting it by the deadline will enter you in competition for scholarships that are awarded by the EHS Dollars for Scholars chapter. These awards include those offered by outside organizations and those offered by the chapter itself.

Complete your sections of the application and give the appraisal form to the person you have selected to do your appraisal. One letter of recommendation is required in addition to the form. It can be from the same person filling out the form or someone different. You are encouraged to select a school or college counselor, administraor, or teacher. Other possible recommenders include employers, members of the clergy,  job supervisors, or any other person who is in a position to evaluate you according to the criteria given. You should supply an envelope to any individual completing an appraisal form or writing you a letter of recommendation. You must print your name and "Dollars for Scholars Recommendation" on the front of the envelope. The appraiser must put the form or letter in the supplied envelope, seal it, sign it across the sealed flap, and return it directly to you to turn in with your completed application packet.

You are responsible for seeing that all supporting documents are submitted. Edgewood Dollars for Scholars will process only applications found to be complete as of the application deadline.

You are also required to submit an essay as part of this application. After submitting your application data online, you should direct your browser to the page for submitting your essay. The form for completing your essay is available online at www.ehs-dfs.org/essay.htm.  We recommend that you copy your essay from a (saved) word processing document into the form by using the Copy/Paste function. You may complete your essay online or, if you prefer, you may hand-deliver a printed copy of your essay to the EHS Guidance Office.

REMEMBER: This application becomes valid only when all of the following have been submitted by the deadline: scholarship application, letter of recommendation; appraisal form; and application essay.
