Applicant Checklist

Include this checklist with your submission of your scholarship application materials to the EHS Guidance Office. Remember, the deadline for all application materials is

___ Scholarship application submitted online or delivered to EHS Guidance Office

___ Scholarship essay submitted online or delivered to EHS Guidance Office

___ Letter of recommendation and appraisal forms (properly labeled and sealed envelope)  delivered to EHS Guidance Office.

Note: You must print your name and "Dollars for Scholars Appraisal/Recommendation" on the front of the envelope. The appraiser must put the appraisal form and/or letter in the supplied envelope, seal it, sign it across the sealed flap, and return it directly to you to turn in to the guidance office.

Be sure to turn in your materials prior to the application deadline, April 2, 2010.
